I know that running a road race is a potentially hazardous activity, which could cause injury or death. I will not enter and participate unless I am medically able and properly trained.
By my signature, I certify that I am in good health, have not been advised by a physician or other medical provider not to participate in this, or a similar event, or physical activity, and I am properly trained for each and all of the events I am entering. (If in doubt as to your physical condition to engage in an event as strenuous as these races, it is strongly recommended that you seek the advice of a competent physician.)
I agree to abide by any decision of a race official, including law enforcement officials, relative to any aspect of my participation in this event, including the right of any official to deny or suspend my participation for any reason whatsoever. I attest that I have read the rules of the race and agree to abide by them.
As a condition to participation in this event, I assume all risks associated with or relating to participation, including, but not limited to: falls; potentially injurious physical contact with other participants, volunteers, race personnel, contract service providers, employees, spectators or objects that will or may be located on the race course, such as barricades or other traffic or crowd control devices, timing mats, aid station tables, or service vehicles; hazardous conditions of the roadway, shoulders and sidewalks or other public areas on and around the race course or pre or post-race event venues; the effects of weather on participants and/or the course (including but not limited to high or low temperatures, rain or wind); any acts of violence that occur at or during the event; and the potential of contracting a communicable disease, including but not limited to COVID-19, associated with contact, or being in close proximity, with other participants, volunteers, race personnel, contract service providers, employees, and spectators.
I understand and acknowledge that Lightning Warriors Hockey Program, Inc ("LWHP") cannot fully protect me from or against the risks associated with this event, including the possible exposure to communicable or infectious diseases. I further agree to abide by the Center for Disease Control’s (CDC) recommendation for the prevention of the spread of COVID-19 and other communicable diseases that are or may be in place as of the date of the event, and I attest to having read the CDC’s current guidance, as applicable.
In consideration of my entry being accepted in the Lightning Warriors St. Patrick's Day 5k Run/Walk, I, intending to be legally bound, do hereby, for myself, my heirs, executors, and assigns, waive, release and forever discharge any and all rights and claims for damages that I may have or which may hereafter accrue to me against LWHP, or any subsidiary or subdivision thereof, it’s or their respective officials, agents, representatives, volunteers, insurers, successors, assigns and event sponsors (the “Released Parties”) from all claims or liabilities of any kind arising out of or relating to my participation in the Lightning Warriors St. Patrick's Day 5k Run/Walk, even though that liability may arise out of negligence or carelessness on the part of the Released Parties.
I understand that bicycles, skateboards, roller skates or inline skates, animals, and personal music players are not allowed in the race, and I will abide by all race rules. I also understand that in the event the Lightning Warriors St. Patrick's Day 5k Run/Walk cannot be held as scheduled because of COVID-19 or another public health event, an act of God, or circumstances beyond LWHP’s control, including the issuance of any executive, county, municipal or judicial order, law or rule, the Lightning Warriors St. Patrick's Day 5k Run/Walk will be canceled and I will be entitled to a partial entry fee refund or partial entry fee deferral to the next year's event.
I grant the Released Parties permission to use my photographs, motion pictures, images, likeness, recordings, or any other record of this event for any legitimate purposes, including for commercial purposes, without compensating me. I also hereby consent to permit emergency treatment in the event of injury or illness. LWHP reserves the right to reject entry for any reason. I understand all persons under 18 years of age must have the written consent of their parent or legal guardian to compete in the abovementioned events. I, the undersigned parent, or guardian, hereby consent to the applicant's participation and waive and release all rights and claims for damages as is more fully set forth above.

Pursuant to section 849.0935 of the Florida Statues, in connection with any raffle or similar offerings, LWHP is providing the following information in a conspicuous disclosure: (a) The rules governing the conduct and operation of the drawing -- drawing will take place in-person, in public view, by drawing paper tickets following the completion of the 5k event each year; (b) The full name of the organization and its principal place of business -- Lightning Warriors Hockey Program Inc., a licensed/certified 501(c)(3) nonprofit under the tax code, with principle place of business at 8030 111th Terrace E. Parrish, FL 34219 (c) The source of the funds used to award cash prizes or to purchase prizes -- donations from local and national supporting organizations and individuals; (d) The date, hour, and place where the winner will be chosen and the prizes will be awarded -- at the 5k location following completion of the annual 5k, prizes to include airlines tickets, autographed hockey memorabilia, and other swag/memorabilia from our generous sponsors to be updated on a regular basis at https://x.com/warriors5k (e) Confirmation that no purchase or contribution is necessary.
